Access to patient records
All patient treatments must be documented in the patient's records. Patients are entitled to receive a copy of their patient records.
To obtain a copy of your records, you must complete the following form:

The records will be sent to the patient by post. The hospital may charge you for this.

We are not permitted to answer medical questions or send copies of patient records by email.
If the content of your email is of such a nature that it requires formal processing, it will be saved in our case and public correspondence archives. Your query will not be processed faster if you send it by email.
