Medikamentell behandling av nasepolyppar – Omalizumab (Xolair) – poliklinisk behandling

Xolair blir blant anna brukt i behandling av kronisk biholebetennelse med nasepolyppar. Medikamentet bidrar til å redusere størrelsen på polyppane og forbetrar symptom som nasetettleik, tap av luktesans, slimsamling bak i halsen og rennande nase.

Xolair inneheld verkestoffet omalizumab. Omalizumab er eit kunstig protein som liknar naturlege protein som blir produserte i kroppen. Det tilhøyrer ein klasse legemiddel som blir kalla monoklonale antistoff. Proteinet hemmar immunmolekylet immunglobulin E (Ige).

Før

Før behandlingsoppstart må du vege deg. I tillegg må du ta ei blodprøve for å sjå på mengda IgE i blodet, såkalla total IgE. Dette er for å avgjere rett dose av medikamentet. Etter dette er det ikkje nødvendig med oppfølgande blodprøver under behandlinga. Sidan dosen blir sett ut frå total IgE og vekt, er det viktig å informere legen dersom vekta di endrar seg drastisk.

Legen skriv ein resept på Xolair til deg som du sjølv hentar ut på apoteket og tar med til poliklinikken. Utover dette krev ikkje behandlinga nokon andre førebuingar.

Dersom du tidlegare har hatt ein allergisk reaksjon (anafylaktisk sjokk) er det viktig at du informerer legen om dette på førehand. Gi også beskjed ved latexallergi, sidan nålhetta inneheld lateks.  

Under

Omalizumab (Xolair) blir gitt i ei ferdigfylt sprøyte som blir sett i underhuda (subcutant). Dosen, og dermed kor mange sprøytar som må settast ved kvar behandling, avheng av total IgE og vekta di.

Dei tre første behandlingane skal alltid skje på ein stad med akutt medisinsk beredskap (legekontor/sjukehus). Deretter kan du sjølv gjennomføre behandlinga heime. Dersom du etter kvart ønsker dette, vil du få opplæring på poliklinikken.

Kor lang tid tar behandlinga?

Legemiddelet blir sett som ei sprøyte og inneber derfor eitt eller fleire nålestikk. Sjølve sprøytesettinga tar berre 5 minutt. Etter første behandling må du observerast i 1 time i avdelinga. Dei to neste behandlingane krev ei observasjonstid på 30 minutt. Dersom du etter tre behandlingar ikkje har hatt nokon reaksjonar, treng du ikkje noka observasjonstid etter at sprøytene er sette.

Oppbevaring

Xolair blir oppbevart i kjøleskap (2–8 grader celcius), men må ikkje frysast. Sprøyta skal oppbevarast i ytteremballasjen for å bli verna mot lys. Før du skal ta legemiddelet, tar du sprøytene ut av kjøleskapet og lèt dei ligge i originalemballasjen i romtemperatur i 15–30 min. Sprøyta må ikkje ligge ute av kjøleskapet lengre enn 4 timar før ho blir sett.

Etter

Når du har fått sprøytene og eventuell observasjonstid er over, kan du reise heim.

Kvar dose blir gitt med eit intervall på 4 veker. Effekten kan sjåast allereie etter 4 veker, men for nokon kan det ta lengre tid. Behandlingsbehov og effekt blir fortløpande vurdert av legen. Dersom du ikkje merkar nokon behandlingseffekt etter 6 månader, vil legen vurdere å avslutte behandlinga.

Oppfølging

Under behandlinga er det vanleg at du blir følgd opp med kontrollar på poliklinikken eller hos fastlegen. Dette blir avtalt med øyre-nase-halslegen.

Ofte stilte spørsmål

Kan eg bruke Xolair om eg er gravid eller ammar?

Per i dag er det ikkje noko som tyder på at denne behandlinga er skadeleg for foster eller barn som ammar. På den andre sida er det heller ikkje etablert med sikkerheit at det er heilt trygt. Det vil derfor trengast ei individuell vurdering i kvart tilfelle.

Kan eg ta vaksinar mens eg står på Xolair?

Det blir anbefalt å unngå å ta levande vaksinar (for eksempel gulfeber, BCG, MMR) mens du står på immundempande medisinar. Skal du ta ikkje-levande-vaksinar som influensavaksine eller covid-vaksine, blir det anbefalt å vente 1–2 veker mellom vaksine og Xolair. Si frå til legen dersom du skal ta vaksine, slik at neste dose med Xolair kan planleggast med tilstrekkeleg opphald.

Du kan ta dei faste medisinane dine som vanleg dersom du ikkje har fått nokon annan beskjed av legen.

Praktisk informasjon

Patients who require overnight accommodation can call our contact centre by phone on 52 73 90 00. Please notify us well in advance.

Hospital hostel

We can offer simply furnished rooms in our hostel, located about 50 metres from the hospital. The room has a direct telephone connection to the surgical monitoring station and also includes an extra bed if you are accompanied by a relative.

The hostel has shared kitchen and bathroom facilities. If you require an evening meal, this must be brought with you.

Keys can be collected from the reception desk, located next to the main entrance on the ground floor of the hospital. The reception is staffed from 07:30 to 18:00, Monday to Friday. If you arrive after 18:00, please ring the doorbell and a member of our staff will assist you.

You will receive an invoice for your stay at the hostel.

Alternative accommodation in Odda

If you have been instructed to have blood tests taken, you can go to the laboratory on the lower ground floor (basement level) at Odda Hospital.

The cafeteria on the 6th floor is available to patients, visitors and staff.

Monday to Friday from 09:30 to 15:30. It is closed on weekends and public holidays.

The patient cafeteria on the 4th and 5th floor is only available for patients.

Many of the wards have their own dining rooms with simple self-service available for patients. Please contact your ward for information.

Kiosk

There is a Narvesen kiosk on the ground floor where you can buy food, sweets, magazines and other items.

You can change your appointment up to 48 hours before your scheduled visit. This can be done through helsenorge.no or by contacting us by phone on 52 73 90 00. Our contact centre is open from 07:30 to 15:30. 

On helsenorge.no, you will find information about your appointment in your appointment letter.

Please check in at the reception desk at the main entrance when you arrive. You can pay at the counter or by using Vipps.

The nearest grocery store is Spar Ådland, Olvikvegen 52, 5416 Stord. It is 1.5 kilometers by car, or a 500 meter walk via the pedestrian path behind the ambulance station.

The nearest grocery store is Bunnpris, located 700 metres from the hospital. The address is Hjøllo 1, 5750 Odda.

The multi-storey car park north of the hospital is reserved for patients and visitors. You must pay a fee to park. 

You can pay by cash or card at the parking machines, or by using the EasyPark app.

There is no number plate recognition at the car park. You must start and stop your parking session yourself using the EasyPark app. 

Prices

The parking fee is NOK 25 per hour. The maximum daily rate is NOK 155.

Haugesund Parkering Drift AS is responsible for managing the car park and the parking areas in front of the main entrance. Any complaints regarding parking fees should be directed to them. Information is available on the parking machines.

There are three charging points available for electric vehicles. 

Reserved parking

Reserved spaces for disabled parking are located outside the main entrance. Please remember to place the parking permit clearly visible in the windscreen. 

There are reserved spaces for blood donors and dialysis patients in the multi-storey car park. These are marked with dedicated signs. If you are entitled to use these spaces, you will be issued with a parking permit by the department where you receive treatment.

It is not permitted to take photos or videos of other patients or hospital staff. Photos or videos must not be shared on social media without the consent of those shown. 

We ask that you respect the privacy of everyone you encounter during your stay at the hospital, and that you only take photos or videos of your own family and friends.

The hospital grounds are smoke-free.

Inpatients who require nicotine products can contact the healthcare staff on their ward.

Please bring any regular medication you take. It may take some time to obtain the same medication from the pharmacy.

Please leave valuables at home. Theft can occur in hospitals.

Bring comfortable clothing and suitable, sturdy footwear. Walking barefoot is not permitted due to hygienic reasons.

Bring toiletries such as a toothbrush, toothpaste, a comb, deodorant and, if needed, an electric shaver. Also bring assistive devices you depend on, for example a walking stick, crutches, wheelchair etc.

Good hygiene is an important factor in ensuring patients receive good care. 

Free wireless internet is available for patients, next of kin and visitors.
