Respiratorisk syncytial-virus (RS-virus)

RS-virus er eit vanleg virus hos både barn og vaksne om vinteren.Vaksne og store barn får i liten grad symptom, og blir fort friske ved RS-virus-infeksjonar. Dette gjeld også dei aller fleste små barn. Hos nokre små barn kan sjukdomen gi infeksjon i dei små luftvegane (bronkiar) eller lungebetennelse. Det gir hoste og pustevanskar. Dette gjeld særleg hos dei yngste under eitt år, og spesielt barn under tre månadar. For nokre få er det nødvendig med innlegging i sjukehus, og for nokon vil det også vere nødvendig med forskjellige typar pustehjelp.

Råd for å redusere smitte av RS-virus

RS-virus smittar gjennom dråpar som blir frigjorde ved at ein person som har infeksjon hostar, nys eller snakkar i nærleiken av deg. Smitte kan også overførast dersom du tar på noko ein smitta person har hosta, nyst eller tatt på, og deretter gnir deg i auga eller tar deg i nasen.
Det er derfor viktig å ha god handhygiene også heime og mellom sjuke og friske barn.
 
Vidare er det anbefalt:
 
  • å holde sjuke barn heime frå barnehage og sjuke vaksne heime frå jobb. Viss det er mogleg, prøv å halde avstand mellom spedbarn og eldre søsken når dei er sjuke.
  • at storesøsken må vaske hender når dei kjem heim frå barnehage eller skule før 
    dei helser på spedbarn i familien
  • at barnehagebarn som er sjuke skal vere heime frå barnehagen til allmenntilstanden er betre og barnet har vore feberfri i minst 24 timar
  • at dei som har små barn, bør unngå besøk av vaksne og barn som har 
    luftvegssymptom 
  • at i familiar med nyfødde og spedbarn, bør ein ikkje ha så mykje besøk heime. Erin skal likevel ta omsyn til at familien kan trenge støtte, omsorg og avlasting
  • at dersom storesøsken blir sjuke, bør foreldre passe på at dei ikkje er tett på 
    spedbarnet, sjølv om det kan vere vanskeleg i praksis. Dette er særleg viktig i 
    dei første dagane av ein luftvegsinfeksjon når du er mest smittsam.
 
Dei vanlegaste symptoma er som ved forkjøling med hoste, nysing og rennande nase. Hos små barn kan sjukdomen gi hoste, pustevanskar og pipande pust. Pusten blir raskare og barnet bruker mykje krefter på å puste. Barnet kan bli bleikt og få blåleg farge rundt leppene. Barnet kan få feber, bli sliten, spise lite og vise mindre interesse. Dei minste barna under 1-2 månadar kan i starten ha pustepausar på 5-10 sekund som einaste symptom. 

 

Tilvising og vurdering

Dersom fastlegen eller legevakten meiner pasienten kan ha behov for innlegging på sjukehus, vil dei tilvise til oss.

Indikasjonar for vurdering på sjukehus: 
      
  • Låg metning (cirka < 92%)
  • Dårleg væskeinntak eller betydeleg pustebesvær (auka respirasjonsfrekvens trass febernedsettande behandling, inndragningar og/eller apnéar)
Barna vil vanlegvis ikkje trenge vurdering ved sjukehuset dersom dei      
  • drikk og et tilfredsstillande og ikkje har symptom på dehydrering
  • har ein oksygenmetning cirka > 92 %
  • ikkje har betydeleg pustebesvær 
      
Det er den kliniske statusen som er avgjerande. Mistanke om RS-virus gir ikkje auka indikasjon for innlegging/vurdering. Terskelen kan likevel vere lågare for å sende dei aller minste (under 2-3 månadar) inn for vurdering, men dei fleste også i denne alderen vil kunne vere heime dersom punkta over er til stades.      
  
  


  


Utredning

På sjukehuset blir det gjort undersøkingar, og det vil mellom anna bli tatt prøvar frå nase eller hals (som ved koronatesting) for å sjå om RS eller eit anna virus er årsaken til sjukdommen. I nokre tilfelle vil det vere aktuelt å ta blodprøvar for å sjå på nivået av CO2 i blodet, og røntgenbilde av lungene. 

Behandling

Det er inga behandling som har effekt mot sjølve viruset, det må kroppen kvitte seg med sjølv. Behandlinga er derfor å behandle plagene ved å hjelpe til med å halde nase og luftvegar opne, tilføre ekstra oksygen viss det trengs, og hjelpe barnet med å spise. I nokre tilfelle får barnet mat gjennom ein tynn slange inn nasen og ned i magesekken (sonde).
 
Viss barnet strever mykje med pusten får dei ekstra oksygen. Vi kan og gje oksygen med noko auka trykk (highflow eller cpap/bipap), som gir enno meir hjelp til å halde luftvegane opne. Er det mykje seigt slim i luftvegane, hender det vi gir inhalasjon med fysiologisk saltvann.


  

 

Oppfølging

Dei fleste barna vil ha hoste som kan vare ei til to veker etter utskriving. Nokre barn treng nokre dagar etter dei har kome heim før dei er like friske som før dei blei sjuke. Dei fleste blir heilt friske og treng ingen spesiell oppfølging i etterkant.

    Vi har eigen inngang til avdelinga som kan brukast når hovudinngangen til sjukehuset er stengt. Gå opp trappa til høgre for det interkommunale legevaktsbygget, og følg gangvegen heilt til døra. Dersom døra er stengt, ringer du på ringeklokka til venstre for døra. Vi kjem og hentar deg.
